Access your Policy
View, print or download (and share with your members) your plan Certificate(s). The following Certificate(s) apply to your plan(s):
Class 01 MEMBER ELECTING DENTAL COVERAGE 
Class 02 MEMBER ELECTING EYE CARE COVERAGE
In order to help you meet ERISA requirements for the delivery of your plan certificate, we offer a report to help you keep track of plan members who access their certificate(s) via our Secure Member website.

In addition to viewing these documents online, you can also Download them as PDF files to access later, or (for the certificate) to attach to an email for your members!
Download Instructions:
- Click on the link to the document you want to Download. The document will open in a separate window.
- Click on the Download icon.
- Select "All" to Download all pages.
- Under the "Download As" list select PDF then click on the Download icon.
- When the Download is complete click on the "Save" button.
- The Save As window should appear. From here you can select where you want to save the file to. Also, you can give the file a unique File Name that will help you in identifying the file later.
