EYE CARE APPEAL PROCESS

The Claim Appeal Procedure is to provide a claimant a reasonable opportunity to appeal the denial of a claim.

  1. If a claim is denied, appeals must be done in writing. You may submit documents, records or other information for consideration in the review. You are entitled to receive reasonable access to and copies of all information used to determine benefits upon request and free of charge. You reserve the right to civil action under ERISA 502(a) if your request for benefits is denied following review of your appeal.
