• Benefits for Preventive, Restorative, Major Restorative & Orthodontia.
• Use any eligible dentist.
• Plan couples benefit percentage allowance, specific reimbursement amounts in one plan and scheduled benefits.
• Reimburses you or your dentist (specific set amount for dental services).
• Employer paid or voluntary; if voluntary, 10 lives or 10%, whichever is greater.
• The Standard and UCR or Scheduled Plan can be combined to form a Dual Option Benefit.
