Ameritas is excited to partner with UNC Health Care to provide a consolidated employee dental program with a choice of two plan options, Freedom of Choice or Network Plan.

Ameritas dental network: Our network includes more than 413,500 dental access points across the nation. Find a dental provider.

In-network vs. out-of-network: Your dental plan provides benefits whether your dentist is in- or out-of-network, giving you the freedom to visit any provider you choose. However, when you select an in-network dentist your out-of-pocket expenses are generally 10-30 percent lower, and there are no claim forms to complete. Network Savings

Dental Rewards^®: You can increase your dental maximum benefit amount via Dental Rewards. Each year you submit at least one dental claim and keep your total claims paid at or below $500, you qualify to carryover $250 in benefit dollars to the following year. If you visit an Ameritas network provider, you earn an additional $100 in rewards.

Maternity benefit: To keep expectant mothers as healthy as possible, the maternity dental benefit provides an additional comprehensive evaluation and cleaning during pregnancy. 6 dental facts for pregnant mom and babies

ID cards: access your personalized ID card via your secure member account, then print it or save it to your smartphone. Discover how online services can help you get the most from your benefits.

Explanation of Benefits (EOB): Receive email EOB's instead of paper statements by selecting “go paperless” via your secure member account. You will get your benefit information faster and minimize your risk of identity theft.

Late entrant: If you decline or drop dental coverage and later choose to sign up after the initial enrollment period, you will be considered a late entrant. As a late entrant, you will be eligible for only exams, cleanings, and fluoride applications for the first 12 months you are covered.
